Coordinating an Appointment for Someone Else

When scheduling for a family member, client, patient, resident, or employee, please include the signer’s name, location, identification status, document type, estimated number of notarizations, facility contact, access instructions, witness arrangements, and payment arrangements.

The signer must personally appear, communicate directly with the notary, and independently agree to proceed.
